Differential Response Case Manager

Provide in-home case management services including implementing client's individual family treatment plans and coordinating services and referrals provided to clients.

Requires:

• Bachelor’s degree in Social Work or a related field and one or two years related experience and/or training or equivalent combination of education and experience
• Valid CA driver’s license and a good driving record
